LiveOak Living Community, established in 2002, is an innovative assisted living facility designed for adults with cognitive disabilities, including mental illness, brain injury, developmental disabilities, and autism. The community is built on the belief that everyone deserves a place to go, activities to engage in, and companionship. LiveOak consists of three locations: LiveOak Ranch, a 63-acre ranch; LiveOak Apartments, a quaint 5-unit complex; and SmithHouse, a 7-bedroom group home. Each location offers tailored support and opportunities for residents to thrive in a caring and inclusive environment.
This community is state-licensed (not a Medicare Care Compare skilled nursing facility). Federal CMS star ratings and PBJ staffing do not apply here.
